HEALTH CONNECTSHEN
Clinic Terms and Conditions
By booking an appointment with Health ConnectShen, you acknowledge that you have read, understood, and agree to the following terms and conditions.
1. Appointment Bookings
Appointments may be booked online, by telephone, email, or in person.
Please ensure that all personal and health information provided is accurate and complete. This information assists your practitioner in providing safe and appropriate care.
2. Cancellation and Missed Appointment Policy
We respectfully request a minimum of 24 hours’ notice for appointment cancellations or rescheduling.
Appointments cancelled with less than 24 hours’ notice, or appointments that are not attended, may incur a cancellation fee of up to the full consultation fee.
Repeated late cancellations or missed appointments may require prepayment for future bookings.
3. Arrival Time
Please arrive on time for your appointment.
Late arrival may reduce your consultation or treatment time to avoid delays for other patients. Full consultation fees may still apply.
4. Acupuncture and Chinese Medicine Care
Acupuncture and Chinese herbal medicine are forms of complementary healthcare designed to support the body’s natural healing processes.
Individual results vary and no guarantees can be made regarding treatment outcomes.
Many conditions respond best to a course of treatment rather than a single session. Your practitioner will discuss treatment recommendations based on your individual circumstances.
5. Chinese Herbal Medicines
If herbal medicines are prescribed:
- Please follow dosage instructions carefully.
- Inform your practitioner of all medications, supplements, allergies, and medical conditions.
- Contact the clinic if you experience any unexpected reactions or concerns.
- Do not alter prescribed medications without consulting your medical practitioner.
Unused herbal medicines are generally not refundable once dispensed.
6. Online Consultations and Herbal Prescriptions
Online consultations rely on the information provided by the patient.
Patients are responsible for providing accurate and complete information regarding their health history, medications, allergies, and current symptoms.
Health ConnectShen reserves the right to decline or postpone treatment if insufficient information is provided or if an in-person assessment is considered necessary.
Postage and delivery timeframes are estimates only and may vary depending on location and postal services.
7. Payment Terms
Payment is required at the time of consultation unless prior arrangements have been made.
Online consultation packages must be paid in full prior to review by the practitioner.
8. Private Health Insurance
Patients are responsible for confirming their eligibility for private health insurance rebates.
Health ConnectShen cannot guarantee rebate eligibility or rebate amounts.
9. Privacy and Confidentiality
Health ConnectShen is committed to protecting your privacy and handling personal information in accordance with applicable Australian privacy legislation.
Personal and health information is collected for the purpose of providing healthcare services and managing your care.
Your information will remain confidential except where disclosure is:
- Required by law;
- Necessary to prevent a serious threat to health or safety;
- Authorised by you in writing.
Please refer to our Privacy Policy for further information.
10. Children and Minors
Patients under 18 years of age must have consent from a parent or legal guardian.
A parent or guardian may be required to attend consultations for younger children.
11. Practitioner Recommendations
Treatment recommendations provided by Health ConnectShen are based on professional clinical judgement and experience.
Patients retain the right to accept or decline any recommended treatment, herbal prescription, lifestyle advice, or referral.
12. Limitation of Liability
To the maximum extent permitted by law, Health ConnectShen accepts no liability for loss, injury, or damages arising from the use of services or products provided, except where such liability cannot be excluded under Australian law.
13. Changes to Terms and Conditions
Health ConnectShen reserves the right to amend these Terms and Conditions at any time. Updated versions will be made available on our website and booking platform.
If you have any questions regarding these Terms and Conditions, please contact Health ConnectShen prior to your appointment.
